Dear New Client,

Welcome to my website! If you are reading this, there is a good chance we have a session scheduled together. I am looking forward to meeting with you, but first there are a few items I would like you to review.

It is important to me that you are well-informed. I really want you to know about important issues such as confidentiality, insurance, fees and the risks and benefits of therapy.

I have included a few handouts and releases for you to review and bring with you to our first session. The entire 7-step process should take about 15-20 minutes or so to complete, but it will save us spending the first session reviewing this information and filling out these forms.

I hope you will find the information contained within this section helpful in making our first session together an informative and efficient one.

I will be happy to answer any questions you have at our initial session together.

Step 1:

Practice Information, Policies and Guidelines: This is 7 pages of information that tells you about my practice and payment policies, general information about therapy and confidentiality issues. I ask that you review this online. You do not need to print it out unless you would like to for your own records.

Step 2:

Signature Page: Please print out this page to sign. Please bring to our initial session.

Step 3:

Intake Form: Please print out this form and fill it out completely.

Step 4:

Notice of Privacy Practices: This is a standard form required by federal law. Please review, sign, and bring in the first page for your file.

Step 5:

Insurance Information Form: If you plan on using your Blue Cross Blue Shield Insurance, please print out this form and fill it out.

Step 6:

Credit Card Authorization Form: Please print and fill out this form for billing purposes.

Step 7:

Release of Information Form: Please fill out a copy of this form for each individual you would like me to be in communication with and informed of your treatment. People you may want to sign a release of information form include current psychiatrists, primary care physicians, dietitians/nutritionists, previous therapists, family members, etc. If there is no one you will need me to be in contact with during your treatment, you do not need to fill out this form. You can fill out a release of information at any point during your treatment.
